Heart and Vision: Kristin Duncan on Leading Safety Culture at Texas Health Resources

Kristin Duncan has spent her career at the intersection of strategy and compassion - believing that the best safety cultures are built not just through rigorous systems, but through the kind of human connection that makes teams want to improve.

At Texas Health Resources (THR), one of the largest faith-based, nonprofit health systems in the United States, Kristin has been instrumental in embedding High-Reliability Organization (HRO) principles across the enterprise - driving measurable progress toward zero preventable harm.

From Bedside to System Change

Kristin's entry into healthcare quality came through direct patient care. That foundation gave her an intimate understanding of where systems succeed and where they fall short - and a lifelong commitment to closing those gaps.

"When you've been at the bedside, you understand why this work matters," she says. "You've seen what happens when safety breaks down. That never leaves you."

Over time, she expanded her focus to the systemic drivers of safety: leadership behaviors, reporting culture, learning systems, and organizational design.

Embedding HRO Principles Across a Large System

At THR, Kristin has led efforts to reduce serious safety events by engaging care teams, identifying actionable opportunities, and consistently measuring performance. The work is as much cultural as it is operational.

"HRO isn't a methodology you install," she says. "It's a mindset you cultivate, one conversation at a time, one team at a time." That belief drives how she approaches everything from executive briefings to frontline huddles.

The Role of Technology and Innovation

Kristin is energized by the role that emerging technologies - particularly AI and natural language processing - can play in transforming patient care. She sees them not as replacements for clinical judgment, but as tools that help providers make better, faster, data-driven decisions.

"The goal is always to support the clinician, not to replace them," she says. "Technology should make it easier to do the right thing."

Leadership That Inspires Change

Kristin's approach to leadership combines technical expertise with interpersonal skills. "Strong communication, empathy, attention to detail, and collaborative leadership are essential," she says. These qualities help her build trust and foster a culture where care teams feel empowered to voice concerns and contribute to safety improvements.

For those new to the field, Kristin emphasizes mentorship, shared learning, and systems thinking. "Celebrate the wins and let the data do the talking," she advises. "Stay focused on collaboration and continuous improvement. Anyone can make a meaningful impact."

When she's not driving change at THR, Kristin enjoys baking - a hobby that reflects her passion for precision. "It combines chemistry, biology, and physics," she explains. "It's a metaphor for life: learn from the process and adjust along the way."
