Hospitals attested to the Patient Safety Structural Measure for the first time this spring, and this fall the scores go public on Care Compare. Most will say “yes” to Domain 3’s high-reliability practices. Far fewer can show those practices have any impact.
The measure is self-attested, so the only safeguard is the hospital’s own integrity and evidence. And the CY 2026 performance year ends December 31 — anything not in place by then cannot be attested next spring.
Attestation is the baseline. Effectiveness is the goal — and the only way to safeguard an attestation is to establish practices that genuinely work.
- Run a mock attestation on Domain 3 and identify which "yes" answers rest on structure that exists on paper but underperforms in practice.
- Select and pressure-test your four counted practices under D3-D against the specific cadence and follow-up requirements, not the headline.
- Redesign leader rounding so each round captures a concern, assigns an owner, closes the loop, and generates its own attestation evidence.
- Apply an honest-audit test to action plans: specific, owned, dated, verified, and tied back to the event or signal that prompted it.
- Build a Domain 3 evidence file organized by statement, with a named owner for each, drawn from artifacts your normal work already produces.
- Interpret your hospital’s first public score in context — total-only display, all-or-nothing domains, lowest score wins under a shared CCN — and brief your board before publication forces the conversation.
